A widely accessible antidepressant, fluvoxamine, offers significant relief for individuals suffering from persistent fatigue due to long-term COVID-19 infections, according to a comprehensive global clinical trial co-led by McMaster University.
Researchers discovered that fluvoxamine (marketed as Luvox), a cost-effective medication commonly utilized for depression treatment, alleviates fatigue and enhances quality of life in adults battling long-term COVID-19 symptoms. This randomized, placebo-controlled trial was published in the Annual Report of Internal Medicine.
Effective Treatment for Long-term COVID-19 Fatigue
Fatigue ranks among the most common and debilitating symptoms reported by those experiencing long-term COVID-19 effects. For many, this fatigue severely disrupts work, family duties, and day-to-day activities. Despite the widespread nature of this issue, effective treatments supported by robust clinical evidence remain scarce.
“This marks a significant advancement for patients seeking evidence-based treatment options,” says Edward Mills, senior author and professor at McMaster University’s School of Health Research Methods, Evidence, and Impact, and co-principal investigator of the trial. “Fluvoxamine has demonstrated consistent benefits and is already widely used and well-understood, indicating its strong potential for clinical application.”
The study was co-led by researchers from Canada, Brazil, and the United States, with clinical sites across the Brazilian states of Belo Horizonte and Minas Gerais.
The REVIVE-TOGETHER trial involved collaboration from McMaster University, the University of British Columbia, Stanford University, the University of Pittsburgh, Duke University, Georgetown University, and various Brazilian institutions.
Fluvoxamine and Metformin Evaluation
The study enrolled 399 Brazilian adults who experienced chronic fatigue for at least 90 days post-confirmation of SARS-CoV-2 infection. Participants were randomly assigned to three groups, receiving fluvoxamine (Luvox), metformin (a standard diabetes medication), or a placebo for 60 days.
“We aimed to assess the efficacy of two existing, widely available, and affordable drugs. We had biological reasons to consider both could effectively counter long-term COVID-19 fatigue, but neither had been thoroughly validated for this purpose in rigorous clinical trials,” Mills explained.
Fluvoxamine outperformed the placebo in reducing fatigue, showing efficacy over placebo 99% of the time. Participants receiving fluvoxamine also reported enhanced quality of life across multiple indicators.
However, the same positive outcomes were not observed with metformin. An earlier study suggested that metformin could decrease the risk of developing long-term COVID-19 complications if taken during the early stages of infection. Yet, in this trial, it did not yield significant improvements for those already suffering from long-term coronavirus fatigue.
Innovative Adaptive Clinical Trial Design
The research team employed a Bayesian adaptive trial design, allowing for the early termination of treatment groups when clear evidence emerged. This method provides reliable conclusions more rapidly than traditional trials while maintaining scientific rigor.
“This trial’s sophisticated adaptive design enabled us to efficiently reach our conclusions, stopping evaluations when sufficient evidence was available. This innovation in trial design is as crucial as the outcomes themselves,” noted lead author Gilmar Reis, a researcher at Card Research, a Brazilian clinical research center in Belo Horizonte, and an adjunct associate professor at McMaster University.
Need for Continued Research
The ongoing impacts of long-term COVID-19 remain a significant global health issue, affecting an estimated 65 million people worldwide. As proven treatments are still unavailable, most medical guidance emphasizes supportive strategies, such as activity pacing and personalized symptom management.
Researchers caution that while fluvoxamine shows promise for fatigue management, it is not a comprehensive solution for long-term COVID-19. This condition encompasses various symptoms and biological processes, necessitating broader research to identify the patient demographics most likely to benefit, understand the drug’s efficacy mechanisms, and explore potential combinations with other developing treatments.
“This trial provides clinicians with the first robust evidence of a pharmaceutical intervention to alleviate long-term COVID-19 fatigue,” said Jamie Forrest, corresponding author and postdoctoral fellow at the University of British Columbia. “Patients are in search of actionable options, and this discovery brings us closer to that goal.”
