In 2010, doctors in Kentucky reported the case of a 2-year-old boy who survived the infection. His illness was identified 10 days after he was admitted to the hospital. Although he experienced significant neurological damage, his condition gradually improved after discharge. At that time, he could not follow commands, communicate, or walk. After 22 months of rehabilitation, he was able to sit upright, smile socially, follow simple commands, and speak.
In this case and most others, it remains unclear exactly how the patient became infected. Balamuthia mandrillaris is an amoeba found in soil and water worldwide, although reported infections occur more often in warmer regions. Researchers believe people may become infected by inhaling amoebas carried in windblown dust or when contaminated soil or water enters an open wound. Experts still do not know why some people develop the infection while others do not. People with weakened immune systems face a higher risk, but otherwise healthy individuals can also become infected, as demonstrated by the child in the recent case report.
Early symptoms of Balamuthia mandrillaris infection commonly include fever, headache, nausea, vomiting, and fatigue. Some patients may also develop noticeable skin lesions, particularly on the face. Neurological symptoms can emerge days, weeks, or even months later and may include confusion, altered mental status, seizures, paralysis, and difficulty walking or speaking.
